Creating a Feedback Form

Forms are a great way to make your website truly interactive by enabling visitors to fill in and submit information or feedback to you.

A form is a collection of form elements or fields on a page that collect information.  A site visitor fill in a form by typing text into fields, clicking a button, checking boxes, or selecting options from drop-down menus.  The visitor then submits the form by clicking a button when the form information is completed.

Creating a feedback form on your website is not at all difficult because the hosting server will provide you with several form formats.  On our website, we are using FrontPage software.  Whenever form information is submitted, the FrontPage appends the form contents to a specified form results file in our web.  We can review the contents of this file periodically to view the information we have collected.  We can also configure a form so the information is sent to us directly via e-mail.
